A £49 consultation plus £19.99 for the fit note is not a £49 consultation. How the two pricing models compare in practice.

Private GP services price in two broadly different ways. Some charge one fee that covers the consultation and anything arising from it. Others charge for the consultation and then separately for each document — the fit note, the referral letter, the repeat prescription.
Neither is dishonest. But the headline price of the second model is not what most people end up paying, and it is worth doing the arithmetic before you book.
On that example, a consultation that produces a fit note costs £68.99. One that produces a fit note and a referral costs £88.98.
You know the cost before you book, and it does not change based on what the consultation concludes.
There is a subtler point. Under per-item pricing, every document has a price attached at the moment it is being discussed. We would not suggest anyone is influenced by that, but removing the question entirely is cleaner — the decision about whether you need a fit note should be a clinical one and nothing else.
It also matters for the consultations that produce several documents at once. A student needing a mitigating circumstances letter and a prescription, or someone needing a sick note and a referral, pays £40 here either way.
If you need only one small thing, per-item pricing is cheaper — sometimes substantially.
A repeat prescription with no consultation at £19.99 is half our fee. If you know exactly what you need, have had it before, and need no assessment, paying £40 for twenty minutes you do not want is poor value and we would rather say so.
Per-item services are also often better set up for that transaction — asynchronous, no appointment to attend, turnaround in hours. Our model assumes you want a conversation. Not everyone does, and not every problem needs one.
Some of those services also deliver medication to your door. We do not — you collect from your own pharmacy.
That last condition is the catch. You rarely know in advance whether a consultation will produce a letter, a fit note, or a referral — because that depends on what is found.
Do not compare headline consultation fees. Compare the total for what you actually needed.
Ask three questions before booking anywhere:
A £49 consultation that becomes £69 with one document is more expensive than a £40 one that does not. Every document arising from a consultation here is included in the £40 — letters, notes, referrals and the prescription itself.
The 20-minute consultation and anything arising from it — the prescription, a fit note, a referral letter, a medical letter, and brief follow-up.
There is no per-document charge.
Laboratory fees for blood tests, and the medication itself, which you pay for at your own pharmacy.

Ask specifically whether letters, fit notes, referrals and follow-ups cost extra. The answer is usually somewhere on the site, and often not on the pricing page.
Brief follow-up messaging is included. A further full consultation for a new problem is a new appointment.

We would rather you did that arithmetic than assume our answer — the difference only appears once documents are involved.
You still pay the £40. The fee is for the assessment, and being told with reasons that you do not need treatment is a legitimate outcome of one.

No. We are not here to replace GPs, only to support them. Your NHS practice remains responsible for your ongoing care, and with your consent we write to them after the consultation. We are useful when you need to be seen sooner, or want more time than a standard appointment allows.
Twenty unhurried minutes and a comprehensive assessment. A written treatment plan is included, along with any prescription, sick note or referral letter arising from the consultation, at no extra charge.
£40 for 20 minutes, self-pay. Membership is optional and never required. Priced for fairness, because we are not here to charge extortionate amounts for access to a doctor.
It means the diagnosis can usually be made from your history and photographs, and that treatment can be arranged safely without examining you. Where a condition is not marked treatable online, it is because examination is the diagnosis — listening to a chest, feeling an abdomen, examining an ear or a joint — or because a procedure or device is needed.
Yes, where there is a genuine bacterial infection — cellulitis, impetigo, bacterial urine infections, confirmed strep throat. We will also frequently tell you that you do not need one. Most sore throats, coughs, colds and earaches are viral, and an antibiotic gives you the side effects with none of the benefit.
If your problem turns out to be genuinely outside what we can do remotely, the doctor will tell you straight away, point you to the right service, and refund your fee in full. We would rather do that than fill twenty minutes to justify the charge.
Call 999 or go to A&E — do not book an online appointment and do not wait for a reply to an email. For urgent problems that are not emergencies, NHS 111 is available 24 hours a day, online or by phone, and can direct you to the right service.
